The film opens with a narration detailing the intricate workings of the human body, home to approximately 37 trillion cells, each with a specialized role. Red blood cells (RBCs) transport oxygen, while white blood cells (WBCs) combat pathogens. Their crucial teamwork ensures human survival. The internal body is depicted as its world, beginning in the red bone marrow, the birthplace of blood cells. Here, hematopoietic stem cells generate various blood cell types. Progenitor cells, destined to become erythroblasts, mature under the watchful care of macrophages. We meet AE3803, a tiny erythroblast, who is startled by a Streptococcus pyogenes. A young WBC, U-1146, promises to protect AE3803 but fails, necessitating intervention from a mature WBC. Later, adult RBCs, including AE3803, undergo an enucleation ceremony, preparing them for their vital role in oxygen transport.
Why is the newbie RBC afraid?
The scene shifts to the real world, where Niko scolds her father for his unhealthy habits. As Niko heads to school, she feels a throat discomfort. We then transition back inside Niko’s body, where AE3803, now a clumsy RBC prone to getting lost, struggles with her duties. Suddenly, Staphylococcus Aureus invades, attacking RBCs. True to his childhood promise, U-1146 arrives to protect AE3803, successfully destroying the bacteria. A siren blares, announcing a pneumococcal bacteria outbreak from Helper T-cell HQ, alerting Killer T-cells. U-1146 explains to the bewildered AE3803 that these bacteria prey on nutrient-rich RBCs like her, threatening organ damage if not eliminated. Unbeknownst to AE3803, she inadvertently carries Streptococcus pneumoniae in a box. The bacteria escape, endangering AE3803 and the human body. Just in time, U-1146 tricks and traps the bacteria in a capsule for destruction, ready to be expelled by a sneeze.
At school, Niko’s crush on a senior (Senpai) triggers an adrenaline rush, leading to a party of dopamine and endorphins inside her body. This surge causes Niko to fall and get a scratch. Now, Nikoโs Senpai comes to her aid. Inside, U-1146 explains the need for platelets to fix the wound. Platelets arrive, initiating coagulation, followed by RBCs and WBCs covering the clot until the wound dries. Meanwhile, Nikoโs father, Shigeru, neglects his health by smoking, drinking alcohol, and following a poor diet. The repercussions manifest within Shigeruโs body as cells struggle against bloodstream toxins. A newbie RBC and a senior RBC work to deliver oxygen. Upon alcohol entering Shigeruโs system, the newbie samples it, prompting the senior to take him to the body’s chemical factory. There, the newbie witnesses an RBCโs demise, consumed by Kupffer cells. Frightened, the newbie is reassured by the seniorโs promise of protection.
What happens to the senior RBC?
At home, Niko texts Senpai, asking him out. To Nikoโs surprise, Senpai agrees to go on a date with Niko. Inside Nikoโs body, WBCs are practicing fighting. Among them is a young boy who looks up to U-1146 and promises to become strong like U-1146. Later, the macrophage alerts everyone that influenza viruses are on the loose. Before the killer T cells can do anything, the NK cells kill many virus cells. NK and killer T cells have a rivalry as NK is free to fight, but killer T cells need to get an order to act. However, the influenza virus is increasing in number. So, U-1146, killer T, and NK fight the pathogens. Later, AE-3803 expresses how inadequate she feels, so U-1146 says that without RBCs delivering oxygen, none of them can do their job. So, RBC’s role is vital.
The next day, Niko and Senpai go on a date. During the date, Niko shares the story of the scarf that her mother knitted while she was in the hospital. Unfortunately, Nikoโs mother died before finishing the scarf, so Shigeru finished it for her. Meanwhile, Shigeru is driving when he has an urgency to poop. Inside Shigeruโs body, the external anus is determined not to let poop get out until the brain sends a message. This is also determined by Shigeruโs determination to hold his poop until he reaches a rest stop. Because of rectal pressure and external anus fight, newbie and senior RBCs are stuck. Finally, Shigeru finds a restroom and excretes. At this time, the newbie is almost sucked out, but the senior saves him. However, Shigeru poops again, so some RBCs are pushed out, and one among them is the senior.
What happens to Niko?
Senpai asks Niko to be his girlfriend, and Niko gladly agrees. During that time, Shigeruโs workplace calls Niko to take Shigeru home as he passes out. At the office, Niko yells at Shigeru for not visiting the doctor for a follow-up. Later, Senpai gives a note to Niko showing Shigeruโs extra work because he wants to save money and educate Niko to get into medical school. Inside Nikoโs body, two young WBCs are called to inform them that they will be destroyed as they have not grown in the last 14 days. One of those boys is the one who looks up to U-1146. Now, the young boy is unwilling to accept this, so he fights and turns into a leukemia cell, killing the WBCs in front of him.
At the same time, AE-3803 is getting better at her job. The platelet leader informs U-1146 that the other platelets are missing. Even NK points out that there aren’t many WBCs around as she fights a pathogen. As Niko is walking to the school gate, Niko faints. Thankfully, Senpai is around, and he takes Niko to the hospital. The doctors inform Shigeru that Niko has leukemia and is prone to bleeding because of a low platelet count. To start the treatment, they will need to do a blood transfusion. Now, Shigeru tells Niko that she will have to undergo chemotherapy. Later, Shigeru sees a blood donation drive and decides to give blood. So, RBCs from Shigeruโs body enter the plastic bag. One among them is the newbie. This same blood is now administered to Niko, and the newbie joins the other RBCs.
What is the treatment plan to beat leukemia?
The RBCs are working hard to provide oxygen to everyone. Killer T, NK, and U-1146 come face-to-face with the leader of the leukemia cells. A report about multiplying leukemia cells distracts the trio, and they head to the lungs and spleen area. When the trio reaches the bone marrow, they realize anti-cancer agents are induced, which kills cancer cells but also healthy ones. Now, NK and Killer T fight the leukemia cells so U-1146 can enter the red bone marrow. Meanwhile, the newbie accepts defeat as he thinks there is no point in delivering oxygen as it is a lost cause. However, AE-3803 explains the importance of their work, and that everyone else is giving it their all, so they should do their job as well.
In the real world, doctors give external oxygen as there is a lack from within. Meanwhile, the leader’s leukemia cells kill the Killer T and NK cells. The Killer T HQ is also taken over by leukemia cells, killing everyone present. Suddenly, AE-3803 faints, and in the real world, the doctors tell Shigeru that they need to do a bone marrow transplant. To do that, they need to kill all the cells inside the bone marrow. Now, the newbie saves AE-3803 from leukemia cells by asking her to supply oxygen to those who are fighting. Thankfully, the macrophage saves the newbie and AE-3803. Just in time, AE-3803 arrives to give oxygen to U-1146. In the real world, Niko is going through radiation therapy, which kills all the cells. Still, U-1146 and AE-3803 escape from this.

U-1146 asks AE-3803 to enter the red bone marrow and exchange oxygen for more reinforcements. As they make their way, the leader leukemia cells interrupt them, asking for oxygen. So, AE-3803 leaves to get reinforcements, and U-1146 fights the leader of the leukemia cell. During the duel, the leader defeats U-1146. Inside the bone marrow, AE-3803 sees that all the stem cells are dead. Now, the leader leukemia cell arrives to kill AE-3803, but U-1146 enters the bone marrow and kills the leukemia leader. The leukemia leader did not realize U-1146 was still alive. At the hospital, Senpai and Shigeru cheer up Niko. Before dying, U-1146 asks AE-3803 to give oxygen to other cells. Niko gets a bone marrow transplant.
After the transplant, a kid’s stem cell runs through the devastated place, bringing it back to life. The new bone marrow is built. Before dying, AE-3803 writes a note revealing her story for the next generation of RBCs. A small girl, RBC RB-2525, finds this note and is inspired. One year later, Niko has recovered and is attending her medical school entrance ceremony with Shigeru and Senpai. Suddenly, Niko sneezes and finds it odd. Inside her body, RB-2525 has now grown up, helping out the platelets. Now, Niko has developed Cedar pollen allergy, so the cells fight it. A new WBC U-8996 arrives to destroy the allergens.
In the end, Niko survives leukemia because of the hard work from RBC, WBC, platelets, and all the fighter cells. Niko is still dating Senpai and also gets into medical school.
